WHO IS THE SERVICE FOR?
People aged 18 and over with substance misuse and alcohol dependency.
HOW DOES IT WORK?
The patient is offered a triage assessment to establish what support is needed. He/she is given a full overview of the service so that they can determine what works best for them. A treatment package is designed to support the patient’s needs.
The Horizons service is not a specialist mental health service or crisis team. The service can signpost to specialist services for issues such as housing benefits, employment etc.
WHERE IS THE SERVICE AVAILABLE?
The service is available at the hubs below or a community setting within the patient’s locality. Home visits are available in exceptional circumstances.

Monday to Friday 9am-5pm.

HOW TO ACCESS THE SERVICE
Referrals are accepted via email, fax or phone via single point of contact. Patients can also drop in to hubs or community venues and self-refer.
